TWO (2) FROM: David H. Ready, City Manager BY: Public Works & Engineering Department SUMMARY This is the final balancing change order for the 2004/2005 Annual Slurry Seal Project, City Project No. 05-02. Total quantities for Schedules A and B were under contract estimate which resulted in a credit to the contract amount. Total quantities for Schedules C, D, E, G, H and I were more than the contract estimate which resulted in an increase to the contract amount. Overall total for all increased and decreased quantities resulted in an increase to the contract amount. RECOMMENDATION: 1) Adopt Minute Order No. "APPROVING CONSTRUCTION j CHANGE ORDER NO. TWO (2) TO AGREEMENT NO. 5053 WITH BOND BLACKTOP, INC., IN THE AMOUNT OF $26,280.20 FOR A TOTAL REVISED CONTRACT AMOUNT OF $766,454.77 FOR THE 2004/2005 ANNUAL SLURRY SEAL PROJECT, CITY PROJECT NO. 05-02." 2) Authorize the City Manager to execute all necessary documents. i STAFF ANALYSIS: i The Public Works Department oversees the maintenance of the City's street system. Each year, the Department assembles a capital project to continue its slurry seal program on various city streets. This year's slurry seal program, was scheduled to include streets within Section 1 (Bid Schedule A), streets within Section 6 (Bid Schedule B), streets within Section 7 (Bid Schedule C), Alejo Road and Avenida Caballeros within Section 14 (Bid Schedule D), streets within Section 27 (Bid Schedule E), streets within Section 34 (Bid Schedule F), streets within Section 35 (Bid Schedule G), Sunrise Way from San Rafael Road to Vista Chino and Ramon Road to East Palm Canyon Drive and from Sunrise Way to El Cielo Road (Bid Schedule H), Ramon Road from Belardo Road to Indian Canyon Drive FTEM NO. L1 Y City Council Staff Report September 7, 2005 - Page 2 CCO#2 for 2004/2005 Annual Slurry Seal and from Sunrise Way to El Cielo Road (Bid Schedule 1), and new traffic striping and markings on several major city streets (Bid Schedule J). Provided slurry seal prices did not exceed the engineer's estimate, over 6,7000,000 square feet of city streets were scheduled for a slurry seal this year. The budget, however, did not allow for all of the Bid Schedules to be awarded and Schedule F (Section 34) was not awarded by City Council. City Council awarded the slurry seal contract to Bond Blacktop, Inc, on March 9, 2005. All slurry street quantities were measured by both City Staff and the Contractor. Final as-built quantities were more than the contract estimate. An additional fifteen (15) working days were added to the contract time due to a delay caused by the REAS aggregate supplier. FISCAL IMPACT: This project was budgeted as part of the 2004/2005 Capital Improvement Plan, with $756,782.42 in Local Measure A Funds. The recommended Construction Change Order No. Two (2) amount is $26,280.20. Sufficient funds are available in Local Measure A account 134-4498-50100 (Slurry Seal Program). David J. Barakian David H. REASON FOR CHANGE: Measurement of square foot quantity was verified by both the City Inspector and the Contractor and the amount represents the increases and decreases for each bid schedule. Additional eighteen(18)working days were approved.2 were weather related, 1 for additional quantities and 15 was for a delay caused by the contractor's aggregate supplier. Liquidated damages will be assessed to the contractor for two (2) working days. All work was completed by July 8, 2005. D.
