Subject: APPROVAL OF AMENDMENT NO. 4 TO CONTRACT SERVICES
AGREEMENT NO. 5235 WITH DOKKEN ENGINEERING FOR THE
INDIAN CANYON DRIVE WIDENING, CITY PROJECT NO. 01-25

SUMMARY
An amendment to the contract services agreement with Dokken Engineering is required
to extend the period of time for construction phase services (contract administration,
materials testing, and inspection) for the Indian Canyon Drive Widening, City Project
No. 01-25.
RECOMMENDATION:
1) Approve Amendment No. 4 to Agreement No. 5235 with Dolkken Engineering, in
the amount of $19,283 for a total revised contract amount' of $970,323 for the
Indian Canyon Drive Widening, City Project 01-25; and
2) Authorize the City Manager to execute all necessary documents.
STAFF ANALYSIS:
On February 15, 2006, the City Council approved Agreement No. 5235 with Dokken
Engineering to provide environmental and civil engineering design services for the
Indian Canyon Drive Widening, City Project 01-25. This project will widen Indian
Canyon Drive from two to four lanes between Tramview Road and the Union Pacific
Railroad, extending through the Whitewater River.
On May 4, 2011, the City Council approved Amendment No. 2 with: Dokken Engineering
in the amount of $190,980 to provide the City with turnkey construction phase services
for this project. The original budget for construction phase services was based on a 90
working day construction period. An additional 39 days were required to complete this
project, and a corresponding budget increase for construction Iphase services was
required.

Amendment No. 3 to Agreement No. 5235
On March 21, 2012, the City Council approved Amendment No. 3 with Dokken
Engineering in the amount of $38,310 for a total revised contract amount of $970,323 to
provide the City with construction phase services for this project. The budget for
construction phase services was based on a 129 working day construction period.
This request for Amendment No. 4 with Dokken Engineering in the amount of $19,283
for a total revised contract amount of $970,323 is based on a 140 working day
construction period. Additional materials testing and additional 11 working days for
construction inspection were required to complete this project.
Staff recommends Council approve the contract amendment which will provide sufficient
budget to close-out this federally funded project.
FISCAL IMPACT:
Sufficient funds for construction phase services are available in account 134-4497-
50216. No City funds are required for this project as CVAG has agreed to cover the
City's local match towards the federal grant paying for construction.

SCHEDULE OF COMPENSATION (Exhibit"C)— Exhibit"C" is amended as follows:
"Phase 3 — Construction Services" is replaced with the attached "Phase 3 — Construction
Services".
