• SECURITY PLAN
All Applicants must submit a security plan.
A security plan that includes the following:
a. Security cameras that have been installed and maintained in good working
condition, and used In an on-going manner with at least 240 continuous hours of
digitally-recorded documentation in a format approved by the City Manager.
Please provide the number of security cameras and list the areas covered by
each. The areas to be covered by the security cameras include, but are not
limited to (1) the storage areas (2) cultivation areas (3) all doors (4) all windows,
and (5) any other areas as determined by the City Manager.
b. A reliable and commercial alarm system that is operated and monitored by a
lawfully-operating security company or alarm business. Please provide the name
and contact information of the security camera monitoring company.
C. Properly illuminated windows and doors that are in compliance with the City's
lighting standards regarding fixture type, wattage, illumination levels, shielding,
etc. (Applicants may be required to secure the necessary approvals and permits.)

All Applicants must submit on one sheet of white paper no less than 11 X 17 inches and
no larger than 24 X 36 inches an accurate detailed site plan and floor plan of the
premises that clearly labels all the uses of areas on the premises, including: 1) doors;
2) entrances; 3) windows; 4) use of each area including 5) storage area(s), 6) cultivation
area(s), 7) exterior lighting fixtures, 8) security cameras, 9) restrooms, 10) signage and
11) parking (including other tenant spaces if the MCCC is proposed for a multi-tenant
building site).
The project shall be required to provide secure bicycle parking facilities on site for use
by residents and commerciallretail patrons and owners. Location and design shall be
approved by the Director of Planning.

Rick Pantele has been a constant proponent of bringing legal medical marijuana
collectives to the city of Palm Springs for over 10 years and assisted the third licensee in
securing its permit while working with them as an advocate and patient liaison dealing
directly with the city, landlords, patients and media during this time. Rick closed all
other business operations on May 15`h• 2015 and will work solely in this endeavor going
forward.
Dr. James Kao
Dr. lames Kao is a licensed physician that has been evaluating patients for medical
marijuana recommendations over the past decade in Palm Springs and the Coachella
Valley. He was the founder and former medical Director of Greensight Medical of Palm
Springs. If granted the permit, he will be no longer be providing recommendations but
will be devoting full time efforts to the safe, responsible, educated usage of medical
• cannabis in the treatment of qualified patients.
Guidelines for Employees and Staff members
a) Enforce a zero tolerance policy for violation of rules or city regulations.
b) Receive ongoing training which examines and addresses all the issues mentioned in
these guidelines.
c) All staff members will be required to complete an extensive training on both product
knowledge, which includes informing and educating patients based on their specific
diagnoses.
d) Additionally staff members are made available at all times to review the personalized
needs of each patient in order to inform them of the different types of benefits and or
potential side effects.
e) Proper scrutiny of doctor recommendations, with the intent of preventing fraud by
• any patient or member of the medical community or both.

PALM SPRINGS PREMIER CARE ASSOCIATES - BASICS
We are a Collective, a Cultivation center, offering delivery service and one of a kind
Hope, Healing and Wellness Center. We are specialist with a serious commitment to
Social Responsibilities to the patients and caregivers.
The Collective will offer the largest selection; strive to keep the cost for the patients
down and increase ease of access (delivery)
The Cultivation Center will be the as eco-friendly as possible, with sustainability safety
accounting being a prime considerations at all times.
The Delivery Service is vital to assisting patients in ease of access and its value also
extends to thee collective which in turn adds increased tax revenue to the city.
The Hope Healing and Wellness Center will be equally important by bringing much
needed sometimes life changing services and support to patients when they need it the
most. These services are not covered by health insurance plans and can make all the
• difference a overcoming obstacles face every day before during and after treatment.
These services may also be offered to the caregivers of patients as they too can be
overwhelmed, experiencing the responsibilities involved.
Our responsibilities include:
a) Ongoing we shall make available to patient's information advising them of the
warning signs of misuse.
b) Restricting (when deemed prudent) the sale of no more than one oz. per client
per day and restricting every patient to no more than two visits per day
c) Maintaining proper surveillance and monitoring of our business grounds and
parking area to prevent violations of membership rules and city regulations.

Price, Affordability and Variety: In both products and pricing levels offered to a wide
range of patient needs. The cost of medications must come down, but not by offering
lesser quality. Levels of pricing will be balanced with a good better best range offered.
Emphasis on Testing and Safety: Through clinical testing for harmful products, strain,
potency and purpose. This is mandatory and will be strictly observed and enforced with
all vendors. In addition to City requirements, we will also conduct our own tests to
monitor THC levels, mold, mildew, fungi and pesticides.
Access and Ease of Purchase: Including delivery service, express service and call
ordering. See Delivery Proposal referenced within
Compassionate Care, Senior & Veteran Center:
Personalized and compassionate care discounts or no cost medications for specifically
approved patients who verify zero or next to no income and are enrolled and medically
• verified. Examples are critically ill patients, hospice patients, terminally diagnosed,
current chemotherapy or last stage AIDS patients.
Veterans consulting specifically for PTSD— We will be offering on site or off site via
telecommunications working with other local Military and Veterans outreach centers
dealing with medical, physiological, family and financial needs. New studies released in
March show PTSD is greatly reduced with medical marijuana. We are located between
29-Palms and Camp Pendleton U.S. Marine bases, the epicenter of, which a new study in
March placed national suicide attempts and completions at close to 50 per day.
Education Center and Ongoing Seminars - Will be offered both on and offsite. These
important services will focus on proper understanding, usage, ingestion and application
of ALL medicines and products. With the growing number of seniors becoming patients
we will offer free seminars (especially in Senior Centers) to insure safety and to expand
knowledge of the many uses of creams and edibles.

a) We will offer customized medication programs devoted to the patient's needs
such as nutrition, anxiety and sleep. See Tinctures and oils
b) There will be a special program for new patients where we will explain and
educate them about the various medications whether they be inhaled, ingested
or topically applied. We will educate the patients about the differences between
indicas, sativas and blends and the psychoactive differences between them.
c) For new patients who are unable to travel, we will offer these programs in the
patient's home or to the patient's care giver.
d) We will offer the most safe friendly easy home delivery service to Palm Springs
and Rancho Mirage. Delivery will be organized, efficiently. Divers will be well
trained and this service will be offered at no cost to the patient (conditions apply).
Delivery hours will be the same as collective hours.
• e) We will work to minimize waiting times for all by offering advance orders,
express pickups and pre-packing for the most popular medications.
e) If zoning were to permit, we would even like to offer our patients the safety
and convenience of a drive up window. This would be no different than what is
currently offered by Walgreens, CVS etc.

590 Oleander Road Palm Springs and 4711 E. Palm Canyon Drive and fully
prepared and ready to immediately upon build out. We will also prepare to begin
delivery immediately upon being permitted and expect to open our collective in
less than 30 days upon receipt of permit.
The finances required to achieve this are in hand. There is no concern of lack of
funding. We are in this for the long haul.
Everything will be designed with sustainability as a priority and in doing so we will
employ a far more sophisticated, safe and secure operation with ease of
verification for tax and regulatory purposes.
We intend to contract Dave Johnston Construction for all work required and it is
important to note that all contactors, vendors, accountants, banking and financial
institutions, paid volunteers, purchases of building materials, office supplies,
security firms, including all specialist providing services in our Wellness Path will
• be new or existing businesses which are all local and to begin first contracting
with Palm Springs based businesses, then Valley based. The money and the jobs
all stay in our town and surrounding cities.
PALM SPRINGS PREMIER CARE ASSOCIATES - HOURS OF OPERATION
Illness and medical needs do not take holidays. Therefore it is our intention to be
open 9:00 AM to 7:00 PM EVERY day as currently permitted under city
regulations.
We will advocate for a later closing time of 9:00 PM allowing sufficient time for
travel for patients living in remote areas, to accommodate work schedules, for
those who for medical or financial reasons cannot drive and must use some form
of public transportation. A reasonable window of opportunity must exist for all.

• PALM SPRINGS PREMIER CARE ASSOCIATES - PURPOSE
If permitted to serve patients in the City of Palm Springs we will bring value to our
patients AND the City of Palm Springs by offering wellness, education and
improved service. We will provide:

Increased City Revenue
We will create and operate the most professional Medical Marijuana Collective in
capacities which will more than exceed the expectations and needs of the patients
while providing an important asset to the community and City of Palm Springs.
We will be more than a collective that dispenses medication and we understand
this will not happen overnight. To recap specifically the Collective we propose will
. hugely impact patients in a positive way with the first priority being lowering
cost of medication and offering delivery.
It will bring huge value to the residents and City of Palm Springs. A key
component is our promise to hiring local, buying local, employing local, getting
involved and thereby making a large financial contribution to our local economy.
We have yet to finalize DBA names at this juncture, but for working purposes we
may consider the use of CannaCare (or other name) for the collective, Buzz
Mobile for the delivery service and TRANSCEND for the Wellness Center.

a) Practice awareness of important concerns that apply to all organizations or
business in which the possibility of misuse, addiction or illegal redistribution exist.
b) Buzzed Driving Awareness Program to be run in association with the County of
Riverside and the Palm Springs Police Department.
c) Be a respectable member of the community, communicate with and
understanding the concerns of all citizens. Operate in a professional capacity at all
times.
d) Always display a professional image in all inter-office and/or public literature,
promotional materials, packaging, required forms, public statements. Seek
advertising in media publications whose readership is geared towards personal
wellness through natural alternatives.
e) Discourage all branding, advertising and promotions which convey an
• improper image such as the use of cannabis leaves or the use of the city name, to
promote products or services. Specifically avoid those designed to attract and/or
falsely promote the use of medicinal medication for non-medicinal purposes.
f) Administrate and operate 100% committed to the PURPOSE OF SERVING THE
COMMUNITY and not the bottom line. Emphasis in education, research,
treatment and safety. Educational seminars to follow. Partner with organizations
such as SC Labs to educate patients about safe use.
g) Maintain an open line of communication with city government and local police
officials, which would include members of the medical community as well as
patients.
h) Engage in an ongoing dialogue which addresses the wellness needs of the
community, while upholding the importance and support the existence of medical
dispensaries that offer natural alternatives to personal and physical wellness.

RICK PANTELE
Rick Pantele is a three time cancer survivor, bone marrow transplant recipient and
26 + year H1V patient. He believes in the science of research and the art of
medicine, by providing a complete range of support services and the most up to
date services and treatments available.
These deal specifically with integrative and alternative techniques including
emotional support, education, nutrition and dozens of other quality of life and
lifesaving needs that are not covered by regular insurance.
He previously helped create the Premiere Oncology Foundation Wellness Program
and has vast hands on experience in this arena. His vision for wellness here in
Palm Springs is based on Hope, Healing, coming from supportive services and is
dedicated to his parents Angelo and Patricia Pantele and his Uncle Gus Pantele all
• who died from lung cancer.
Dr. JAMES KAO
Dr. James Kao is a long-time advocate for responsible medical marijuana usage
having been in the Palm Springs and Coachella Valley for over a decade. Dr. Kao is
a physician and surgeon that is Board-Certified and Fellowship-trained and has
evaluated, examined and treated many patients in the Community.
Dr. Kao had two medical degrees (D.O. and M.D.) and a Master's in Public Health
(M.P.H.) Additionally he has completed additional coursework in Osteopathic
manipulation, acupuncture, medical massage, health-care administration,
herbology, and management.
His father Deh-liao Kao is a former Chief of Police for the United Nations
headquarters in New York City and Southeast Asia. His mother is Nancy Kao, a
• practicing Acupuncturist and former mid-wife and registered nurse with many
years of experience in Neonatology, Pediatrics and Obstetrics.

You have requested an officer six (6) days a week from 10am-6pm. That is a total of 8 hours
per day, 48 hours per week, 2496 annually. The regular billing projection would be $41,184 or
$3432 per month. If you are open on the 7 holidays listed below the bill rate would be at
$24.75 per hour and we would adjust the billing to meet that.
Our rate of service would be $16.50 per hour. We invoice twice per month, the 1st and 15th.

• OPTION AGREEMENT
This Option Agreement (the "Agreement") is made and entered into this ] day of
October, 2015, by and between John Cockrell & Associates ("Seller") and Dr. James Kao
("Buyer"). Seller and Buyer are sometimes referred to herein singularly as a "Party" and
collectively as the"Parties."
RECITALS
A. Seller is the owner of that certain real property known as 590 Oleander Road, Palm
Springs, California 92264 and more particularly described in Exhibit A attached hereto (the
"Property").
B. Buyer has applied to the City of Palm Springs for a permit to operate a medical
cannabis cooperative or collective (the"Permit"); and
C. Buyer wishes to acquire from Seller, and Seller has agreed to grant to Buyer, an
option to purchase the Property on the terms and conditions contained herein.
NOW THEREFORE, in consideration of the mutual covenants contained herein, and
other good and valuable consideration, receipt and sufficiency of which are hereby
acknowledged,the Parties agree as follows:
• 1. GRANT OF OPTION.
Seller hereby grants to Buyer an option (the "Option") to purchase the Property pursuant
to this Agreement.
2. OPTION CONSIDERATION.
A. Deposit. As initial consideration for the grant of the Option, Buyer shall
pay to Seller, via cash or money order, concurrently with the execution hereof, the sum of
Ten Thousand Dollars ($10,000.00) (the"Option Deposit"). The Option Deposit shall be
nonrefundable in the event that Buyer does not exercise the Option.
B. Option Purchase Price. Upon exercise of the Option, Buyer shall pay to
Seller, via cash or money order, the amount of One Hundred and Fifty Thousand Dollars
($150,000.00) as full consideration for the Option ("Option Purchase Price"). The
Option Deposit shall be applied to the Option Purchase Price following the exercise of
the Option.
3. TERM OF OPTION.
The term of the Option shall commence as of the date of this Agreement, and shall expire
• at 11:59 p.m., local time on November 30, 2015 (the "Option Term") unless otherwise mutually
agreed to by the Parties.

collectives to the city of Palm Springs for over 10 years and assisted the third licensee in
securing its permit while working with them as an advocate and patient liaison dealing
directly with the city, landlords, patients and media during this time. Rick closed all
other business operations on May 151h, 2015 and will work solely in this endeavor going

Dr. James Kao
Dr. James Kao is a licensed physician that has been evaluating patients for medical
marijuana recommendations over the past decade in Palm Springs and the Coachella
Valley. He was the founder and former medical Director of Greensight Medical of Palm
Springs. If granted the permit, he will be no longer be providing recommendations but
will be devoting full time efforts to the safe, responsible, educated usage of medical
• cannabis in the treatment of qualified patients.
Guidelines for Employees and Staff members
a) Enforce a zero tolerance policy for violation of rules or city regulations.
b) Receive ongoing training which examines and addresses all the issues mentioned in
these guidelines.
c) All staff members will be required to complete an extensive training on both product
knowledge, which includes informing and educating patients based on their specific
diagnoses.
d) Additionally staff members are made available at all times to review the personalized
needs of each patient in order to inform them of the different types of benefits and or
potential side effects.
e) Proper scrutiny of doctor recommendations, with the intent of preventing fraud by
. any patient or member of the medical community or both.

We are a Collective, a Cultivation center, offering delivery service and one of a kind
Hope, Healing and Wellness Center. We are specialist with a serious commitment to
Social Responsibilities to the patients and caregivers.
The Collective will offer the largest selection; strive to keep the cost for the patients
down and increase ease of access (delivery)
The Cultivation Center will be the as eco-friendly as possible, with sustainability safety
accounting being a prime considerations at all times.
The Delivery Service is vital to assisting patients in ease of access and its value also
extends to thee collective which in turn adds increased tax revenue to the city.
The Hope Healing and Wellness Center will be equally important by bringing much
needed sometimes life changing services and support to patients when they need it the
most. These services are not covered by health insurance plans and can make all the
difference a overcoming obstacles face every day before during and after treatment.
These services may also be offered to the caregivers of patients as they too can be
overwhelmed, experiencing the responsibilities involved.
Our responsibilities include:
a) Ongoing we shall make available to patient's information advising them of the
warning signs of misuse.
b) Restricting (when deemed prudent) the sale of no more than one oz. per client
per day and restricting every patient to no more than two visits per day
c) Maintaining proper surveillance and monitoring of our business grounds and
parking area to prevent violations of membership rules and city regulations.

potency and purpose. This is mandatory and will be strictly observed and enforced with
all vendors. In addition to City requirements, we will also conduct our own tests to
monitor THC levels, mold, mildew, fungi and pesticides.
Access and Ease of Purchase: Including delivery service, express service and call

Compassionate Care, Senior & Veteran Center:
Personalized and compassionate care discounts or no cost medications for specifically
approved patients who verify zero or next to no income and are enrolled and medically
verified. Examples are critically ill patients, hospice patients, terminally diagnosed,
current chemotherapy or last stage AIDS patients.
Veterans consulting specifically for PTSD— We will be offering on site or off site via
telecommunications working with other local Military and Veterans outreach centers
dealing with medical, physiological, family and financial needs. New studies released in
March show PTSD is greatly reduced with medical marijuana. We are located between
29-Palms and Camp Pendleton U.S. Marine bases, the epicenter of, which a new study in
March placed national suicide attempts and completions at close to 50 per day.
Education Center and Ongoing Seminars - Will be offered both on and offsite. These
important services will focus on proper understanding, usage, ingestion and application
of ALL medicines and products. With the growing number of seniors becoming patients
we will offer free seminars (especially in Senior Centers) to insure safety and to expand
knowledge of the many uses of creams and edibles.

a) We will offer customized medication programs devoted to the patient's needs
such as nutrition, anxiety and sleep. See Tinctures and oils
b) There will be a special program for new patients where we will explain and
educate them about the various medications whether they be inhaled, ingested
or topically applied. We will educate the patients about the differences between
indicas, sativas and blends and the psychoactive differences between them.
c) For new patients who are unable to travel, we will offer these programs in the
patient's home or to the patient's care giver.
d) We will offer the most safe friendly easy home delivery service to Palm Springs
and Rancho Mirage. Delivery will be organized, efficiently. Divers will be well
trained and this service will be offered at no cost to the patient (conditions apply).
Delivery hours will be the same as collective hours.
e) We will work to minimize waiting times for all by offering advance orders,
express pickups and pre-packing for the most popular medications.
e) If zoning were to permit, we would even like to offer our patients the safety
and convenience of a drive up window. This would be no different than what is
currently offered by Walgreens, CVS etc.

prepared and ready to immediately upon build out. We will also prepare to begin
delivery immediately upon being permitted and expect to open our collective in
less than 30 days upon receipt of permit.
The finances required to achieve this are in hand. There is no concern of lack of
funding. We are in this for the long haul.
Everything will be designed with sustainability as a priority and in doing so we will
employ a far more sophisticated, safe and secure operation with ease of
verification for tax and regulatory purposes.
We intend to contract Dave Johnston Construction for all work required and it is
important to note that all contactors, vendors, accountants, banking and financial
institutions, paid volunteers, purchases of building materials, office supplies,
• security firms, including all specialist providing services in our Wellness Path will
be new or existing businesses which are all local and to begin first contracting
with Palm Springs based businesses, then Valley based. The money and the jobs
all stay in our town and surrounding cities.
PALM SPRINGS PREMIER CARE ASSOCIATES - HOURS OF OPERATION
Illness and medical needs do not take holidays. Therefore it is our intention to be
open 9:00 AM to 7:00 PM EVERY day as currently permitted under city
regulations.
We will advocate for a later closing time of 9:00 PM allowing sufficient time for
travel for patients living in remote areas, to accommodate work schedules, for
those who for medical or financial reasons cannot drive and must use some form
of public transportation. A reasonable window of opportunity must exist for all.

promote products or services. Specifically avoid those designed to attract and/or
falsely promote the use of medicinal medication for non-medicinal purposes.

COMMUNITY and not the bottom line. Emphasis in education, research,
treatment and safety. Educational seminars to follow. Partner with organizations
such as SC Labs to educate patients about safe use.

officials, which would include members of the medical community as well as
patients.
h) Engage in an ongoing dialogue which addresses the wellness needs of the
community, while upholding the importance and support the existence of medical

RICK PANTELE
Rick Pantele is a three time cancer survivor, bone marrow transplant recipient and
26 + year HIV patient. He believes in the science of research and the art of
medicine, by providing a complete range of support services and the most up to
date services and treatments available.
These deal specifically with integrative and alternative techniques including
emotional support, education, nutrition and dozens of other quality of life and
lifesaving needs that are not covered by regular insurance.
He previously helped create the Premiere Oncology Foundation Wellness Program
and has vast hands on experience in this arena. His vision for wellness here in
Palm Springs is based on Hope, Healing, coming from supportive services and is
dedicated to his parents Angelo and Patricia Pantele and his Uncle Gus Pantele all
who died from lung cancer.

Dr. James Kao is a long-time advocate for responsible medical marijuana usage
having been in the Palm Springs and Coachella Valley for over a decade. Dr. Kao is
a physician and surgeon that is Board-Certified and Fellowship-trained and has

Dr. Kao had two medical degrees (D.O. and M.D.) and a Master's in Public Health

Because of community support from individuals such as you, we are able to raise funds
to provide help to the children in need of the Coachella Valley.
In 2010, we donated three Sunshine Coaches that will help service the Boys & Girls
Clubs of Coachella Valley and Cathedral City, as well as the YMCA of Palm Desert.
Variety gave away 300 brand new bikes and helmets to well-deserving 4th graders
throughout the Coachella Valley, thanks to the KPLM/ Variety Radiothon held on Nov.
2nd and 3 d. For the first time in Tent 66, we presented a first grader with Cerebral Palsy
his own adaptive bike through our Mobility Program initiated this year.
We also work with youth groups throughout the Coachella Valley, sponsoring various
arts programs and sports programs. This year, Palm Desert High School Dance Team,
Coachella Valley High School Athletic Department, Desert Hot Springs Boys & Girls
Clubs, and others have benefited from our donations.
With the help of our membership, our fundraisers, and our volunteers, Variety at Work
continues to distribute Easter Baskets and Thanksgiving food bags to Cielo Vista Early
Childhood Development Program.

The staff and students of the Palm Springs Unified School District would like to salute Rick Pantele for his
extensive efforts and generosity in providing a high quality and unique experience in the Fine Arts to the students
of the District.
"Reach" is a powerful oil painting that communicates the possibilities a person has to reach for their own
potential despite obstacles. During a discussion on how to make this painting available to the art departments for
each school in Palm Springs Unified School District, Rick's response was, "Let's not just donate a print; let's take
the actual painting on the road to the students!"
Rick's vision kept expanding, and by the time the painting was ready to travel the District, there were 15 works of
art, along with the three other artists on board.This group visited middle and high schools providing a total of 12
separate art shows. Rick enlisted community support and underwrote the balance of the expense personally.The
artists volunteered their time,and handled all of the manual labor involved in setting up and breaking down the
shows.
The response, like the name of Rick's gallery, was "incredible."The students were thrilled to see the paintings in
such an unusual and personal venue.The artists'stories were captivating, and the students were able to ask
questions and interact with Rick and the other artists one-on-one. Rick had special cards printed with a picture of
the "Reach" painting to hand out to each student.Additionally, he donated a copy of the"Reach" lithograph to
each site, so that the schools would have a momento of the visit. Due to the generosity of Rick Pantele, students
who would never have dreamed of such an experience had the impossible become a reality.
This was quite an unusual undertaking which had a tremendous impact on the District's students. For the serious
art students, it was especially inspirational and informative. For all attendees,the show spoke to the power of
imagination and creative thinking. Rick was unfailingly enthusiastic and generous throughout the entire process.
He is changing lives through his gift of making art the bond that ties.
