This supplemental staff report provides additional information regarding Business Principal
Disclosure as to Martha's Village & Kitchen, Inc.
BUSINESS PRINCIPAL DISCLOSURE:
According to the California Secretary of State website, Martha's Village & Kitchen, Inc., is an active
Domestic Non-profit California Corporation.
According to information provided Martha's Village & Kitchen, Inc., Chief Executive Officer and
President, Linda Barrack, Martha's Village & Kitchen, Inc., Management Officers consists of the
following: Chairman of the Board, Henry Burdick, Pharmative Corp., Co-Founder, Retired,
Chairman & CEO; Treasurer/Vice Chairman, Bill DeMucci, Consultant; and Secretary, Cynthia
