HomeMy WebLinkAbout18725 - RESOLUTIONS - 11/1/1995RESOLUTION NO. 18725 OF THE CITY COUNCIL OF THE CITY OF PALM SPRINGS, CALIFORNIA, APPROVING AGREEMENTS FOR GROUP HEALTH INSURANCE, DENTAL INSURANCE, LONG-TERM DISABILITY INSURANCE AND CITY CONTRIBUTION RATES ON BEHALF OF THE GENERAL MANAGEMENT (GMX), POLICE MANAGEMENT ' (PMX), FIRE MANAGEMENT (FMX), EXEMPT EMPLOYEES AND CITY COUNCIL MEMBERS FOR THE 1994-95 and 1995-96 FISCAL YEARS WHEREAS Resolution No. 18247, adopted on December 1, 1993, approved agreements for group health insurance programs for fiscal year 1993-94, with rates effective through December 31, 1994; and WHEREAS The Public Employees' Medical and Hospital Care Act provides a variety of group health insurance options and has proposed no rate increases; and WHEREAS The City self -insures a "fee for service" dental plan and has proposed no rate increase effective September 1, 1994; and WHEREAS Denticare provides a "pre -paid" dental plan and has proposed a rate increase effective September 1, 1994; and WHEREAS Vision Service Plan provides the administration for the City's self -funded vision benefit and has proposed a rate increase effective October 1, 1994; and WHEREAS, Canada Life Insurance Company provides Long Term ' Disability Insurance for the City's General Unit (GU), Confidential Employees, General Management (GMX), Police Management (PMX), Fire Management (FMX) and Exempt (EX) employees. NOW THEREFORE BE IT RESOLVED by the City Council of the City of Palm Springs as follows: Section 1. Resolution Nos 18243, 18244, 18245 and 18246, provide for the election and participation in the Public Employees' Medical and Hospital Care Act and the group health insurance options provided through the Act, shall be made available to all General Management (GMX), Police Management (PMX), Fire Management (FMX), Exempt employees and City Council Members, for the period of August 1, 1994 through July 31, 1995, and August 1, 1995 through December 31, 1996, at the following monthly rates: SINGLE TWO-PARTY FAMILY FHP, INC. $ 157.50 $ 315.00 $ 409.50 HEALTHNET $ 148.00 $ 296.00 $ 384.80 MAXICARE $ 150.00 $ 300.00 $ 390.00 PACIFICARE $ 156.77 $ 313.54 $ 407.60 BLUE SHIELD HMO $ 156.00 $ 312.00 $ 406.00 AETNA HEALTH CARE $ 156.86 $ 313.72 $ 406.80 PERS CARE $ 256.00 $ 512.00 $ 666.00 PERS CHOICE $ 157.00 $ 314.00 $ 408.00 KAISER $ 153.87 $ 307.74 $ 400.06 PORAC $ 197.40 $ 348.62 $ 489.62 Section 2. A City self -funded dental insurance program, for a "fee for service" dental plan, made available to all City employees and City Council Members, except for Police and Fire Safety Unit employees, for the period of January 1, 1994 through December 31, 1995. A Group services agreement with Denticare, for a R18725 Page 2 "prepaid" dental insurance plan, made available to all City employees and City Council Members, except for Police and Fire Safety Unit employees, for the period September 1, 1994 through December 31, 1996 is hereby approved, at the following rates: 911/94 - 8/31 95 9/1/95 - 12/31/96 Single $ 10.28 $ 10.80 Two Party $ 17.80 $ 18.70 Family $ 23.34 $ 24.50 The City will contribute the following monthly amounts toward premiums for the Denticare dental plan for the covered months of September 1994, through December 31, 1996, inclusive, consistent with the Memorandum of Understanding for all employee units of representation as follows: 9/1/94 -• 8/31/95 9/1/95 - 12/31/96 Single $10.28 $10.80 Two Party $17.80 $18.70 Family $23.34 $24.50 Section 5. An insurance contract with Canada Life Insurance Company for Long Term Disability replacing prior coverage through Hartford Insurance Company, effective October 1, 1994 through September 30, 1996, is hereby approved at a premium rate of $.85 per $100 covered payroll for all General Unit (GU), Confidential Employees, General Management (GMX), Police Management (P'MX), Fire Management (FMX), and Exempt (EX) employees. Section 6. The City will contribute the following monthly amounts toward the administration fee for the City's self -funded vision care benefit for the covered months of October 1, 1994 through September 30, 1996, inclusive, as well as reimburse the monthly benefit payments, consistent with the Memorandum of Understanding for all employee units of representation as follows: Single $2.52 Two -Party $2.52 Family $2.52 ADOPTED this lstday of November, 1995.
