OF THE CITY COUNCIL OF THE CITY OF PALM SPRINGS,
CALIFORNIA, APPROVING AGREEMENTS FOR GROUP HEALTH
INSURANCE, DENTAL INSURANCE AND LONG-TERM DISABILITY
INSURANCE, AND CITY CONTRIBUTION RATES ON BEHALF OF
ITS GENERAL UNIT AND CONFIDENTIAL EMPLOYEES FOR
THE 1993_ -94FISCAL YEAR.
WHEREAS Resolution No. 17939, adopted on September 2 , 1992,
approved agreements for group health insurance programs for fiscal
year, 1992-93 , with rates effective through August 30, 1993 ; and
WHEREAS Blue Shield has proposed a HMO, "pre-paid"' medical benefit
plan effective December 1, 1993; and
WHEREAS Blue Shield has proposed a 11PPO/Fee for Service Indemnity
Plan" effective December 1, 1993 ; and
WHEREAS The City self-insures a "fee for service" dental plan and
has proposed a no rate increase effective September 1, 1993; and '
WHEREAS Denticare provides a. "prepaid" dental plan and has proposed
a rate increase effective September 1, 1993; and
WHEREAS Vision Service Plan provides the administration for the
City's self-funded vision benefit, and has proposed no rate increase
effective September 1, 1993 ; and ,
WHEREAS Hartford Insurance Company provides Long Term Disability '
Insurance for the City's General Unit (GU) , Confidential Employees,
General Management (GM)() , Police Management (PMX) „ Fire Management
(FMX) and Executive (EX) employees; and 4
WHEREAS the City of Palm Springs provides mandatory short-term 1
disability insurance for General. Unit and Confidential employees
only.
NOW THEREFORE BE IT RESOLVED by the City Council. of the City of
Palm Springs as follows:
Section 1. A group service agreement with Blue Shield HMO, for a
"prepaid" health care plan, made available to all General Unit
employees, for the period of December 1, 1993 through December 31,
1994, is hereby approved at the following monthly rates: ,

Section 2. A group service agreement with Blue Shield, for a
t1PPO/Fee for Service Indemnity Plan, made available to all General
Unit employees, for the period of. December 1, 1993 through December ,
31, 1994, are hereby approved, at 'the following monthly rates:

Section 3 . A City self-funded, dental insurance program, for a
"fee for service" dental plan, made available to all City employees
and City Council Members, except: for Police and Fire Safety Unit
employees, for the period September 1, 1993 through August 31,
1994, is hereby approved, at the following rates:: `

Section 4. A group service agreement with Denticare, for a
"prepaid" dental insurance plan, made available to all City
R employees and City Council Members, except for Police and Fire
Safety Unit employees, for the period September 1, 1993 through
August 31, 1994 is hereby approved, at the following rates:

Section 5. An insurance contract with Hartford Insurance Company
for Long Term Disability continues for the period February 1, 1993
through January 31, 1995, is hereby approved at a premium rate of
$.94 per $100 covered payroll for all General Unit (GU) ,
F Confidential Employees, General Management (GMX) , Police Management
(PMX) , Fire Management (FMX) , and Exempt (EX) employees.
Section 6. A Mandatory employee self-insured short-term
disability insurance, with no contribution from the City, provided
to General Unit and Confidential Employees at a monthly premium of
r 0.26% of gross regular base salary fox• covered employees for the
period July 1, 1993 through June 30, 1994, is hereby approved.
Section 7. The City will contribute up to and not to exceed, the
following monthly amounts toward premiums for Blue Shield HMO and
Blue Shield Insurance Company PPO/Fee for Service Indemnity Plan
health care plans for the covered months, previously specified, for
fiscal year 1993/94, inclusive, consistent with the Memoranda of
' Understanding for all employees in each of the respective employee
units of representation as follows:

Section 8. The City will contribute the following monthly

Section 9. The City will contribute the following monthly amounts
towards premium for the City self-funded vision care benefit for
the covered months of September, 1993 through August, 1994,
inclusive, consistent with the Memorandum of Understanding for all

Section 10. The City will contribute at a rate of $.94 per $100 of ,
covered payroll for General Management, Police Management, Fire
Management and Executive employees, General Unit and Confidential
employees towards the premiums for Long-Term Disability Insurance
for the months of September, 1993 through August, 1994, inclusive,
for all employees covered by this insurance.
