OF THE CITY COUNCIL OF THE CITY OF PALM SPRINGS,
CALIFORNIA, APPROVING AGREEMENTS FOR GROUP HEALTH
INSURANCE, DENTAL INSURANCE AND LONG-TERM DISABILITY
INSURANCE, AND CITY CONTRIBUTION RATES ON BEHALF OF
ITS EMPLOYEES FOR THE 1990-91 FISCAL YEAR.
WHEREAS Resolution No. 16817 approved agreements for group health
insurance programs for fiscal year 1989-90, with rates effective
through July 31, 1990 ; and
WHEREAS Pacificare, Inc. provides a "prepaid" health care plan and
has proposed a rate increase effective August 1, 1990; and
WHEREAS The City self-insures two medical benefit plans known as
Palm Springs Health Network and Palm Springs Choice II and has
proposed a rate increase effective September 1, 1990 ; and
WHEREAS Blue Shield Insurance Company provides a "fee for service"
health care plan for Police and Fire Safety Unit employees; and
WHEREAS The City self-insures a "fee for service" dental plan and
has proposed a rate increase effective September 1, 1990; and
WHEREAS Denticare provides a "prepaid" dental plan and continues at
the rates currently in effect; and
WHEREAS Prudential Insurance Company provides a "fee for service"
dental plan for Police and Fire Safety Unit employees; and
• WHEREAS Hartford Insurance Company provides Long Term Disability
Insurance for the City's General Unit (GU) , Confidential Unit,
Management (MX) and Executive (EX) employees; and
WHEREAS the City of Palm Springs provides mandatory short-term
disability insurance for General Unit and Confidential employees;
and
NOW THEREFORE BE IT RESOLVED by the City Council of the City of
Palm Springs as follows:
Section 1. A group service agreement with Pacificare, Inc. for a
"prepaid" health care plan, made available to all City employees,
except Police and Fire Safety Unit employees, for the period of
August 1, 1990 through July 31, 1991, is hereby approved at the
following monthly rates:

Section 2 . The City, self--insured medical benefit plans; Palm
Springs Health Network, which provides a 100% network plan, and
Palm Springs Choice II, which provides, a "fee for service" benefit
plan made available to all City employees, except Police and Fire
Safety Unit employees, for the period of September 1, 1990 through
August 31, 1991, is hereby approved, not to exceed the following
monthly rates:

Section 3 . A group service agreement with Blue Shield Insurance
Company, for a "fee for service" benefit plan, made available to
all Police and Fire Safety Unit employees, for the: period August 1,
1990 through July 31, 1991, is hereby approved, not to exceed the
following monthly rates:

Section 4 . A City self-funded dental insurance program, for a
"fee for service" dental plan, made available to all City
employees, except Police and Fire Safety Unit employees, for the
period September 1, 1990 through August 31, 1991, is hereby
approved, at the following rates:

Section 5. A group service agreement. with Denticaxe, for a
"prepaid" dental insurance plan, made available to al.l. City
employees, except Police and Fire Safety Unit employees, for the
period September 1, 1.990 through August 31, 1991, is hereby
approved, at the following rates :

Section 6. A group service agreement with Prudential Insurance
Company, for a "fee for service" dental plan, made available to
Police and Fire Safety Unit employees for a period of August 1,
1990 through July 31, 1991 is hereby approved, not to exceed the

Section 7 . An insurance contract with Hartford Insurance Company
for a Long-Term Disability Insurance program for the period July 1,
1990 through June 30, 1991, is hereby approved at a premium rate of
$1. 69 per $100 of covered payroll for all Management (MX) and
Executive (EX) employees and a premium rate of $1. 49 per $100 of
covered payroll for all General Unit (GU) and Confidential
employees.
Section 8 . A mandatory self-insured short-term disability insur-
ance will be provided to General Unit and Confidential Employees.
The monthly premium shall be 0. 26% of gross regular base salary for
covered employees for the period July 1, 1990 through June 30,
1991 .
Section 9 . The City will contribute up to and not to exceed, the
following monthly amounts toward premiums for Pacificare, The City
Self-Funded Palms Springs Health Network and Palm Springs Choice II
and Blue Shield Insurance Company health care plans for the covered
months, previoualy specified, for fiscal year 1990/91, inclusive,
consistent with the Memoranda of Understanding for all employees in
each of the respective employee units of representation as follows:

Section 10. The City will contribute the following monthly
amounts toward premiums for the City self-funded dental plan,
Denticare and Prudential Insurance Company dental plans for the
covered months of August, 1989, through July, 1990, inclusive,
consistent with the Memorandum of Understanding for all employee
units of representation as follows:

Section 11. The City will contribute at a rate of $1. 69 per $100
of covered payroll for Management and Executive employees and at a
rate of $1. 49 per $100 of covered payroll for General Unit and
Confidential employees towards the premiums for Long-Term
Disability Insurance for the months of July, 1990 through June,
1991, inclusive, for all employees covered by this insurance.
