i DATE: January 19, 2000 TO: City Council FROM: City Engineer APPROVE CONSTRUCTION CHANGE ORDER NO. 4 TO G & M CONSTRUCTION RECOMMENDATION: That the City Council approve Construction Change Order No. 4 to G & M Construction, for an increase of$ 32,300.00 for the Underground Storage Tank Removal and Replacement, City Project No. 95-17. BACKGROUND: On January 21, 1998, the City Council awarded G & M Construction the contract for the Underground Storage Tank Removals and Replacement project. This change order consists of the following work: Schedule E - Wastewater Treatment Plant (Location No. 5) At the recommendation of the Wastewater Treatment Plant Director and the approval of the Assistant City Manager, remove and replace 3 in. thick pavement and remove broken concrete curb to provide more equipment parking, north of the proposed storage tank location. This work will be part of the proposed paving for the above storage tank. One additional working day will be added to the contract for this work. At the recommendation of the Wastewater Treatment Plant Director and the approval of the Assistant City Manager, install a telephone conduit for the existing sewer lift station located at north east corner of Sonora Road and El Cielo Road. This extra work is part of the upgrade the Wastewater Treatment Plant is doing to communicate with the lift station from the Wastewater Treatment Plant for monitoring purposes. Two working days will be added to the contract for this work. At the recommendation of the Assistant City Manager, construct 3 in. thick asphalt pavement over 3 in. crushed miscellaneous base along the south side of the Airport, near the fueling facilities. This area will be 12 ft. by 270 ft. In addition, 8,000 sq. ft. of Type II slurry seal, 3 layers, will be installed in this area also. Five working days will be added to the contract for this work. 245 additional working days days will be added to the contract due to contamination found during the removal of the existing underground storage tanks located at the City Yard and City Hall locations. Contamination found at the City Hall location has been corrected and accepted by the County of Riverside Health Services Agency, Department of Environmental Health, Hazardous Materials Management Branch. Contamination found at the City Yard location has been corrected and is currently in the process of being accepted. Funds are available in account numbers 420-6820-43200 (WWTP Contractual Services) and 540-5904-48622 (Loss Containment).
