APPROVING PERSONAL SERVICE CONTRACT
BETWEEN THE CITY OF PALM SPRINGS AND
CHARLES J. SUPPLE, M.D. , FOR A PERIOD
OF ONE (1) YEAR COMMENCING JULY 1,
1991 AND ENDING ON JUNE 30, 1992.
I HEREBY CERTIFY that this Minute Order, approving personal
service contract between the City of Palm Springs and Charles
J. Supple, M.D. , for a period of one (1) year commencing
July 1, 1991 and ending on June 30, 1992, was adopted by
the City Council of the City of Palm Springs, California,
in a meeting thereof held on the 3rd day of July, 1991.
