Per our phone conversation, I am enclosing a brief synopsis of
the activities that have taken place thus far, in regard to
locating a railroad station in Palm Springs.
There is a great deal of excitement concerning this subject in
conjunction with the announcement of the casino and the
revitalization of the downtown area. I believe that you will see
the advantages of locating the station in Palm Springs.
Please review the enclosed information and call me if you have

Establishment of the Palm Springs TranscontinentallInter-City Train Station
City of Palm Springs - November 18, 1993
OBJECTIVE: To establish a train station in Palm Springs to be used initially as an Amtrak stop
and eventually as one of the Metrolink stations connecting the Coachella Valley to the Los
Angeles basin.
RATIONALE: Locating a train station in Palm Springs is an important component toward
achieving the goal of intermodal transportation for our region. The site we have chosen is
located near the intersection of Interstate 10 and Highway 62, with convenient access to the Palm
Springs Regional Airport and future Indian gaming casinos in downtown Palm Springs. The
creation of a Palm Springs Train Station would provide a centrally located transportation triad
of roadway -runway -railway to population centers in the Coachella Valley, the high desert region
and the growing Banning -Beaumont area.
The City Council of Indio has endorsed the construction of a Palm Springs station to
supplement their existing Amtrak stop. Two stations would serve the population -heavy "ends"
of the Coachella Valley. Palm Springs and then Indio are, by far, the two largest cities in the
Valley.
The name and reputation of Palm Springs are known world-wide and the addition of
Indian gaming will only serve to enhance our image of being the Monte Carlo of the West. It
is certain that the Palm Springs Train Station would quickly become a popular resort stop
bringing an economic boost to the Coachella Valley.
ACTIONS TAKEN: The City of Palm Springs has applied for state transit capital improvement
funds in the amount of $1,820,000.00 for the construction of a train station. The California
Transportation Commission will make final determination on the allocation of funds by April of
1994. Their evaluation will be based on recommendations made by Caltrans Division of Rail
and the Riverside County Transportation Commission. This application procedure was preceded
by research and contact with Amtrak and Southern Pacific officials as follows:
1) On October 7, 1991, Mayor Sonny Bono and City Manager Rob Parkins traveled
to Washington D.C. to meet with Mr. Tim Gillespie, the Assistant Vice -President
of Government and Public Affairs for the National Railroad Passenger
Corporation. The idea of a Palm Springs station was well received.
2) On July 22, 1992, Mr. James Adams of Amtrak and Mr. David Hampton
representing Southern Pacific met with our Assistant city manager to choose a site
for the station.
3) Jim Robertson of Southern Pacific and Ron Scalaro of Amtrak provided letters
of support to be included in our application for Transit Capital Improvement
funds.
REQUEST: Since submittal of the application, the City of Palm Desert has also applied for the
same funds to build a station in their city. It is clear that there is insufficient money to fully
fund installations in both cities during the same year.
We are requesting that you urge Amtrak officials to endorse the approval of our
application to the deciding agencies according to the above stated rationale.

colleagues in Washington on April 12. 1 was particularly
gratified by the opportunity to personally assure you that Amtrak
continues to support local efforts to develop a station stop
within the city of Palm Springs. I know this was a concern
raised in your March 2 letter. As I indicated in our meeting,
having resolved the two issues of site acquisition and easement,
and with the construction of a station imminent, Amtrak stands
ready to serve the city by scheduling the Sunset Limited (Trains
1 and 2) to stop at Palm Springs.
At our meeting, I also indicated that we would be happy to
discuss the possibility of operating additional service. The
fact that Palm Springs is on a state -designated corridor offers
the prospect of additional rail passenger activity. However, as
we discussed, any such additional service would have to be worked
out in conjunction with the state.
Please do not hesitate to contact me if you have any further
questions. I look forward to hearing about your station
development and the establishment of service to Palm springs.

Train Station Project Overview
City of Palm Springs

PROJECT DESCRIPTION:
The City of Palm Springs is proposing the construction of an Intercity Rail Station at milepost
588.1 of the Southern Pacific Yuma main line within the Coachella Valley. The site is
approximately one-half mile south of Interstate 10 and west of Indian Avenue and would be
accessed via Garnet Station Road.
The station will serve the existing Amtrak "Sunset Limited" and Amtrak Thruway buses. The
"Sunset Limited" provides transcontinental Superliner service between Los Angeles -New
Orleans -Miami three days per week. Based on existing levels of service operated by Amtrak
within the corridor, it is estimated that the station would be patronized by approximately 90
intercity rail persons per week.
The project will consist of the construction of the platform, shelters and parking in order to
implement Amtrak passenger service. Specifically, this will include a platform for Amtrak trains
with lighting, a surface parking lot and lighting for 40 spaces, two or three cantilevered
passenger waiting shelters, 2 bus/RV stalls and associated landscaping. All construction will
take place within the Southern Pacific Easement.

RECOMMENDATION:
That the City Council approve a resolution authorizing a loan
Of $1.150 Million from the City's Wastewater Treatment Plant
fund in order to provide a 50% match to the Transit Capital
Improvement grant for the construction of a train station.
BACKGROUND:
An application for a grant to implement a train station
program in the amount of $2.3 Million was submitted to the
California Transportation Commission in September 1993. Since
then, the City of Palm Desert has also applied for the same
grant funds to build a station in their city. Caltrans staff
recommended fully funding the Palm Desert application in the
amount of $2.3 million.
Recognizing the importance of two train stations to the
Coachella Valley, both cities and C.V.A.G. reached a
compromise which provides the grant funds be split between
Palm Springs and Palm Desert. After hearing this proposal,
the California Transportation Commission asked both cities to
amend their applications and resubmit them for consideration.
In order to meet the eligibility requirements, the City of
Palm Springs must meet provisions set forth for recipients
providing a 50% match.
As proposed, the City of Palm Springs will receive $1.150
Million to be applied towards the estimated $2.3 Million cost
of site acquisition (including easements) and construction of
the train station. The additional $1.150 Million will come
from our matching funds. As part of the eligibility
requirements for the grant, the City must adopt a resolution
to be forwarded to the California Transportation Commission
stating that the matching funds required for this project are
available and have been committed. Furthermore, we are
required to show that the committed funds have been set aside
for the project and are available for expenditure.
Resubmittal of amended application is due at Riverside County
Transportation Commission on March 11, 1994.
Funds are available through a loan from the Wastewater
Treatment Plant fund with a loan interest rate of 5% for a
period of 30 years, payable annually, commencing one year

Dear Mr. Remen:
On February 23, 1994, the cities of Palm Springs and Palm Desert requested the California
Transportation Commission (CTC) consider splitting the $2,288 million of TCI funding (Fiscal
Year 1994/95) recommended for the City of Palm Desert. Based on the testimonies which were
given at the hearing, the CTC directed the applicant agencies to develop a revised financial plan
and schedule, which would split the funds between the two cites.
The enclosed package has the full support of both applicant agencies. Enclosed is the
justification and revised funding plan for the sharing of the funds, as well as a revised
application. If approved for TCI funding, the City of Palm Springs is fully committed to
provide a 50 % local match in the amount of $1,144, 000.00 as delineated in the revised funding
plan. In order to demonstrate its local commitment to the project, the City Council adopted the
attached resolution at its March 9, 1994 meeting.
We appreciate the opportunity your agency has provided both cities in allowing us to prepare
this joint, revised funding plan. The Coachella Valley is extremely desirous of increasing
transportation mobility within the region. The development of two facilities within the Valley
achieving this objective.

The City of Palm Springs is requesting SB 2800 approval for advance local match expenditure
according to California Transportation Commission Resolution #G-91-23. This request is in
accordance with the allocation of $1,114,000 in Transit Capital Improvement funds as
appropriated by the California Transportation Commission on March 30, 1994 resolution #G-94-
08 for the construction of a commuter rail station.
As part of the project description submitted to the C.T.C. and subsequently approved as C.T.C.
resolution #G-94-08, two parcels of land were to be purchased by the City of Palm Springs for
use in the overall train station project. The City is requesting SB 2800 approval to spend
$299,705.78 for the purchase of these properties and have this amount included as part of the
Transit Capital Improvement fund allocation.
Attached is a copy of the City Council resolution approving the purchase of this land.

We are very pleased to support the efforts of the' City of Palm
Springs in developing a railroad station, which will permit Amtrak
to offer service. to the Coachella Valley. Southern Pacific
Transportation Company has been working closely with the City of
Palm Springs for over a year in bringing this new development to
reality.
SPTCo is negotiating with the City of Palm Springs -for
acquisition of railroad owned property to assist in construction
and operation of the proposed station. We recommend funding of
their application to assist them in this endeavor.

Once a project has been approved or programmed, information and forms for grants administration requests such as
allocation requests, SB 580 review, Fund Transfer Agreements and bond certification should be requested by writing to the
Department's District Offices, and requesting the Transit Fund Administration Guidelines.
For each item in this application (Form A -- Application), the information required is discussed in the document,
"Instructions - Completing a Uniform Transit Application" (Form A -- Instructions). The Instructions indicate which items
need to be completed depending upon the programs involved and type(s) of funding requested, the type of mass transit
project and activities involved, and the California Transportation Commission action requested. The applicant should
always refer to the Commission's adopted guidelines and policies, as well as applicable statutes, upon which this
application is based.
Section I, Items 1 and 2, includes the applicant's assurances that eligibility requirements are met and that title and
certification requirements are met. This section once completed can be used again for future applications of the same
project, providing no changes have occurred between application requests, with the proviso that Item 2-p must be changed
to reflect the current date of the subsequent application request.

If your agency's policy board has delegated, by resolution, to the agency's general manager, executive director or
chief executive officer (CEO) the general authority to commit the agency to legally binding commitments with the
state, you should submit a copy of the resolution delegating the authority to the CEO.

The following list of required resolutions and certifications may then be checked off by the applicant that the CEO
is authorized to provide assurances that the conditions, requirements, or statements of fact shall be honored by his
agency's policy board. If the CEO does not have agency delegation the agency is required to provide a resolution
on the eligibility requirements below:

organization has the financial and institutional ability to implement the project and that your organization is
empowered to: let a contract; to sue or be sued by another entity or person; and other responsibilities and
duties of your agency.
X That this project will be available to the general public, or its primary purpose will be to benefit the public
and does not benefit a private entity or individual. If it does not benefit the public, please explain, and
attach your explanation to this application, as part of your submittal. (State funds, in most cases, may not
be used for private passenger rail facilities.)
That the matching funds required for this project are available and committed to this project. Committed
funds have received necessary authorizations and the recipient agency has authority to expend the funds.
This is an intercity project, 50% local match not required under guidelines.

Section I - Eliaibility and Title Certification
1. Eliaibilit
See Attachment 1 for a copy of the resolution authorizing the City
Manager to execute agreements on behalf of the City. Please note
that the City of Palm Springs is prepared to provide whatever
additional documentation is required to meet all the eligibility
requirements specified in the Form A application. At the time of
this application submittal, however, the instructions were unclear
if each of the items listed on Page 2 needed a separate resolution
or if the resolution provided in Attachment 1 was sufficient.
As required, the City is willing to provide any additional
documentation listed in Form A as follows:
a. Statement defining the organizations' financial and
institutional ability to implement the project,
b. The project will be available to the general public,
c. That the City will comply with the Commission's Hazardous
Waste Identification and clean-up Policy for Rail Right -of -
Way.
With regards to the item referring to the use of other funds to
backfill cost increases if the project exceeds state funds
available, the City requests further clarification of this item in
order to determine the City's position.

4. Project Description and Benefits
4.1 Project Description
a.
The City of Palm Springs is proposing the construction of an
Intercity Rail Station at milepost 588.1 of the Southern Pacific
Yuma main line within the Coachella Valley. The site is
approximately one-half mile south of Interstate 10 and west of
Indian Avenue. The site would be accessed via Garnet Station Road.
Although the site is located currently in an unincorporated region,
the site is included as part of an annexation package by the City.
The City estimates the annexation process will be complete by early
1994.
The station will serve the existing Amtrak "Sunset Limited." The
"Sunset Limited" provides transcontinental Superliner service
between Los Angeles -New Orleans -Miami three days per week. The
City has already obtained approvals from both the Southern Pacific
Transportation Company and Amtrak regarding the location of the
site as referenced in Attachments 2 and 3.
Based on existing levels of service operated by Amtrak within the
corridor, it is estimated that the station would be patronized by
approximately 90 intercity rail persons per week. It is also
anticipated that additional patronage would be generated by
stopping the Amtrak Thruway feeder buses at the Palm Springs
Intercity station. Currently, these buses terminate in Indio.
Specifically, the project includes the construction of a 14 x 700
foot platform with lighting, a surface parking lot and lighting for
281 spaces, a 240 foot passenger waiting shelter, 18 bus/RV stalls
and associated landscaping. The station will be constructed on a
15-acre parcel, which the City intends on acquiring during the
first quarter of 1994. A 50 x 700 foot easement must also be
obtained from the Southern Pacific in order to construct the
platform.

The City intends on developing the parcel in phases. Phase I would
consist of the construction of the station, platform, shelter, and
parking. Phase II would include the development of supportive
facilities, including a car rental agency and other
commercial/retail facilities.
The City is requesting AB 3090 approval from the California
Transportation Commission in the amount of $130,000.00 to be
expended during the fourth quarter of Fiscal Year 1993/94. These
funds would be expended as follows:

Because the proposed station would serve the existing Amtrak
"Sunset Limited" route and Thruway buses, the proposed project is
not dependent upon the completion of any additional services to be
fully operational. Additional planning studies of the corridor
which are currently under active review by the State (as referenced
in Item 4.1d) would only increase the station's use if approved for
programming by all relevant agencies.
Palm Springs is a center for desert resorts and recreational
activities. Almost 50% of the hotel rooms in the Coachella Valley
are located within the City. It is expected that hotel room sales
will continue to grow as the desert's tourist industry is marketed
on an international basis by the Palm Springs Desert Resorts
convention and Visitor Bureau. Bureau estimates indicate that two
million individuals visit the Coachella Valley each year.
The permanent population of the City of Palm Springs in 1991 was
42,050, a 30% increase from 1980. During the winter peak season,
another 35,000 people live in the City. By 2000 and 2010, the City
is projected to grow to almost 60,000 and 75,000 respectively.

schematic. Attachment 6 provides a local map of the project site.

d. In addition to the existing, Amtrak "Sunset Limited" service in
the corridor, the Riverside County Transportation Commission has
completed two separate passenger rail feasibility studies of the
corridor. The first report was completed in December 1991 and is
entitled "Los Angeles -Coachella Valley -Imperial County Intercity
Rail Feasibility Study". This report assessed the technical
aspects of operating daily intercity service between Los Angeles
and the Coachella Valley with an Imperial County extension.

Caltrans Division of Rail staff is currently refining patronage and
revenue projections developed in this study and expects to issue a
programming decision regarding the service later this year.
The second study undertaken by the Riverside County Transportation
Commission reviewed the feasibility of providing a limited,
demonstration level of service between Los Angeles and the
Coachella Valley on weekends only during the peak tourism season of
November - May. A draft of this report was released in August 1993
and is entitled "Los Angeles -Coachella Valley Weekend Demonstration
Passenger Rail Service." This report assumes the use of Metrolink
commuter rail equipment owned by the Riverside County
Transportation Commission.
The development of a station within the Palm Springs/Western
Coachella Valley vicinity supports the need for a station in both
studies. It is critical to note, however, that the development of
the Palm Springs Intercity Rail Station is not dependent on the
actual implementation of either of the planned services to be fully
operational.
e. A 50 x 700 foot easement will need be obtained from Southern
Pacific for the development of the platform. As referenced in the
letter from Southern Pacific (shown as Attachment 2), negotiations
are currently being initiated between the City and the Railroad for
the required easement.
f. With regards to the property for the development of the
station, the City of Palm Springs is fully prepared to indemnify
the State of California pursuant to the Commission's Hazardous
Waste Identification and Clean-up Policy for Rail Right -of -Way (G-
91-2). The City is also prepared to provide documentation to the
Commission that full due diligence will be exercised in acquiring
the land and that no additional State funds will be "sought for
clean-up, damages or liability costs associated with hazardous
wastes."
As described in Item 4.le above, the City is proposing that a
"surface rights" only easement be obtained from Southern Pacific
for the construction of the platform. As part of its negotiations
with the Railroad, the City intends on insuring that its interests
are limited solely to the surface of the land and that it will not
enter into any agreement unless the City and the State are fully
indemnified against all potential subsurface contamination.
g. The City of Palm Springs will own the station and assume
responsibly for its ongoing maintenance and operations. It should
be referenced, however, that since construction of the station
represents only the first phase of the total development of the 15-

acre parcel, that the City fully intends on pursuing joint
public/private development activities as described in Item 4.la for
the remainder of the site to offset some of the ongoing station
maintenance costs. The City also intends on approaching the major
hotels and hospitality -related businesses for financial assistance
in both operating the station and providing shuttle services
to/from the site.
h. Bicycle facilities will be included in the final site plan.

4.2 Project Benefits
a.
The development of a Palm Springs Intercity Rail Station would
bring passenger rail service to one of the most renown cities
within the State of California and provide a new transportation
alternative to the City's residents and guests. The Palm Springs
Station would represent the first, "full-scale" passenger rail
station in the Coachella Valley and provide the back -bone for
future service expansions.
As stated in Item 4.1a, the site is situated just south of
Interstate 10, the interstate highway linking Los Angeles, San
Bernardino, and Riverside Counties to the State of Arizona. In
addition, the site is located approximately five miles northwest of
the Palm Springs Regional Airport. Given this location, the site
is ideally situated to maximize connecting transportation
alternatives (i.e. Amtrak Thruway buses, airport shuttles, and
local transit) and provide a link between passenger rail, air
travel, and local feeder/distribution services.
b.
Construction of the Palm Springs Intercity Rail station will
increase access to the "Sunset Limited" corridor from both a local
and regional perspective. Currently, the only stop between Indio
and Los Angeles is the Pomona station. While the Indio station is
also located within the Coachella Valley, it is positioned 22.8
miles east of the proposed Palm Springs site. As shown in the
Table 1, a Palm Springs station would provide an intermediary stop
on this transcontinental route in a city which is recognized

nationally as a popular destination.
A Palm Springs station also fulfills the requirement for a terminal
in western Coachella Valley as recommended in the Dec.1991
Feasibility Study should a favorable programming decision be made
by the Division of Rail.

e. It is assumed that the project will demonstrate cost -
efficiencies because of the fact that the station will serve not
only passengers for the "Sunset Limited," but also passengers for
the Amtrak Feeder Bus System. In addition, pending a determination
by the Division of Rail, the station would serve as one of the
three proposed sites recommended in the Dec. 1991 feasibility
study.
Another cost efficiency of this project is that State funding will
primarily only be used for construction of the site. Because the
City of Palm Springs will be acquiring the land for the station
through non -State resources, the TCI allocation requested in this
application will only be used to fund actual design/engineering and
construction costs. Only a small portion of the State funds will
be used to obtain the Southern Pacific easement.
In addition, while the City of Palm Springs is committed to fund
100% of the ongoing operating costs of the station, the City also
fully intends on offsetting some of the station's operating
expenses through financial assistance from the private sector,
including the hotel and hospitality industries. Pending the full
build -out of the site in Phase II, the City expects to receive
lease revenues which would also be used to offset ongoing station
maintenance and operating costs.

5. Overall Proiect Schedules
The schedule shown on Page 10 of Form A assumes AB 3090 approval
from the California Transportation Commission for station
design/engineering and the Southern Pacific easement. Should this
not be approved, the schedule would be delayed by approximately
four months.
6. Environmental Documentation
The City intends on filing a negative declaration as shown on Page
11 of Form A.
7. System Characteristics
a. Not applicable.
b. Not applicable.
C. As stated in question 4.1.a, a minimum of 90 persons are
estimated to utilize the station per week. This minimum estimate
is based solely on existing ridership patterns of the "Sunset
Limited." The "Sunset Limited" makes three round trips per week.
It was assumed that 15 passengers would board/alight each train for
a total of 90 passengers per week (15 persons x 6 trains).
Additional station usage is also anticipated from the Amtrak
Thruway buses and the station's increased proximity to the hotels
and resorts located within the City of Palm Springs and neighboring
jurisdictions.
In addition to the minimum levels of use estimated from the
passengers of the "Sunset Limited," the Dec. 1991 Los Angeles -
Coachella Valley -Imperial County Intercity Rail Feasibility Study
projected that a station located within the western region of the
Coachella Valley would generate 208 daily boardings and alightings
during its first year of operation; 319 by the tenth year. Should
a determination be made by the State to program this service, the
use of the station would increase accordingly.

will average $100,000.00 annually. This includes the hiring of one
full-time security/maintenance worker with benefits, daily sweep

clearing of blowsand with attending crew, electric/water, and
contingencies (Attachment 8). It is assumed that these costs would
increase by an incremental 5% each year as shown in Item 7.e.

The capital costs associated with the construction of the station

The platform/lighting costs depicted are based on an average of
construction costs for Metrolink commuter rail platforms during the
past 18 months. Site development and passenger shelter costs were
developed by the City of Palm Springs. Land acquisition costs are
based on discussions the City has held with the land owner and
comparable land values.
Preconstruction costs reflect 5% of the total project capital
costs.

SunLine Transit Agency is in support of awarding the Transit Capital Improvement Grant to
be used to build a rail station in the City of Palm Springs.
SunLine is the Joint Powers Authority for the Coachella Valley to provide public transit to the
nine cities, which are Cathedral City, Coachella, Desert Hot Springs, Indian Wells, Indio, La
Quinta, Palm Desert, Palm Springs and Rancho Mirage and Riverside County.
Rail service is an important component towards the goal for intermodal transportation for our
region. Today we have bus, taxi and air transportation to serve 250,000 residents and over
three million visitors.
A rail station is a critical economic issue and will result in job creation. It enhances access to
the Coachella Valley, thereby, supporting our number one industry, tourism.
To link a rail extension to the Coachella Valley, SunLine is prepared to provide public transit
and coordinate taxi service and be of ongoing support as the facility service levels evolve.

Riverside County Transportation Commission to indicate our intention to
pursue a MetroUnk Station in our community. To this end, the City Council
has directed staff to apply for funding to facilitate development of the
station .
It's obvious to us, that Metrolink Stations located in both Indio and Palm
Springs will serve the needs of commuters in the most efficient manner.
We consider it critical to the Coachella Valley that both Palm Springs and
Indio Railroad Stations become a reality. Thuse stations would stimulate
growth and bring new economic opportunities by providin- a direct commuter

Dear Lloyd,
We are indeed pleased at the marvelous progress being made by City star as we strive to get a
railroad station online for Palm Springs. Needless to say, this is an important step as we take
control of our destiny, and at the same time, provide a major shot in the arm for the overall tourist
and visitor industry.
We are most anxious to see a temporary site in place which would facilitate an exhibit by Metrolink
on October 24th in conjunction with the upcoming TRANSCON 2000, October 25 and 26. At the
same time, this is an excellent and important stepping stone toward a demonstration project which
hopefully could begin at the end of this year, providing Metrolink service from Riverside to Palm
Springs. It goes without saying that we are most anxious to see this demonstration project move
forward, and toward that end, are supportive of such steps as may be necessary as we go forward in
an effort to request transit capital improvement funds and such other funding as may be necessary
to make this become a reality.
Clearly, this is a three phased effort with the first being the ability to provide a platform and a site
for Metrolink as an exhibit for TR.ANSCON on October 24. Phase two is the improvement of that
site to facilitate the demonstration project which hopefully can be in place by the end of the year.
Phase three is the upgrading of that temporary facility into a full -service, permanent facility which
would allow not only Metrolink service, but an Amtrak stop as well.
As you are aware, I am certain, Palm Desert is moving forward with a view toward establishing a
railroad station in their community. I would hope that that effort focuses on the need for a third
railroad station as part of the Riverside County Transportation Committee`s intercity rail study,
and not be seen as a competitive effort as we attempt to get this demonstration project with
Metrolink online.
Be assured of our support as we move forward with this most important opportunity.

Once a project has been approved or programmed, information and forms for grants administration requests such as
allocation requests, SB 580 review, Fund Transfer Agreements and bond certification should be requested by writing to th
Department's District Offices, and requesting the Transit Fund Administration Guidelines.
For each item in this application (Form A -- Application), the information required is discussed in the document,
"Instructions - Cumpieting a Uniform Transit Application" (Form A -- Instructions). The Instructions indicate which items
need to be completed depending upon the programs involved and type(s) of funding requested, the type of mass transit
project and activities involved, and the California Transportation Commission action requested. The applicant should
always refer to the Commission's adopted guidelines and policies, as well as applicable statutes, upon which this
application is based.
Section I, Items 1 and 2, includes the applicant's assurances that eligibility requirements are met and that title and
certification requirements are met. This section once completed can be used again for future applications of the same
project, providing no changes have occurred between application requests, with the proviso that Item 2-p must be changes
to reflect the current date of the subsequent application request.
Section 11, Items 3 through 10, should be updated as appropriate with each subsequent application request.
